Develop an implementation plan and explore funding for a universally accessible home visitation system delivered through a cohesive mixed-delivery model.

2. Intake & Referral System

Create a coordinated intake and referral system for home visitation in Jeffco, including navigators, accessible through technology and in-person.

3. Workforce Support

Recruit, retain, and support a qualified home visitation workforce to meet the diverse needs of children and their families.

To improve referrals and cross-program collaboration, the Home Visitation Collaborative is focused on relationships and partnerships by building trust across programs through home visitor connection events and meetings. In 2024, the group hosted an event for 70 home visitors to get to know each other’s programs, strengthen referrals and share best practices. Roundtable discussions resulted in a list of actionable goals for 2025:

Goal 1: Improve cross-program referrals through improved information sharing and processes.

Goal 2: Provide social support and connect families to information about early childhood resources through cross program community gatherings with parents and home visitors.

Goal 3: Enhance home visitor professional development and cross-program learning through shared training and learning spaces for inter-program problem solving.
