Cornerstone 2: Family Support & Home Visiting Programs

The Bright Futures Roadmap is a community-wide strategy to support the health and well-being of our community’s youngest children and families. The Roadmap is organized around four cornerstones or key strategies. Partners have begun to identify opportunities for cross-cornerstone collaboration.

CORNERSTONE 2: Families are supported in their role as a child’s first and most important teacher with family support services that meet their diverse needs.

Cornerstone 2 has three primary strategies, which you can read more about on this page:

  1. Develop an implementation plan and explore funding for a universally accessible home visitation system delivered through a cohesive mixed-delivery model.
  2. Create a coordinated intake and referral system for home visitation in Jeffco, including navigators, accessible through technology and in-person.
  3. Recruit, retain, and support a qualified home visitation workforce.

The Home Visitation Collaborative (HVC) is working on two goals under Strategies 1 & 2 in 2024 to better address families’ needs.

Strategy 1

Develop an implementation plan and explore funding for a universally accessible home visitation system, delivered through a cohesive mixed-delivery model.

2023-2024 Partner Activities

  • Family Connects Demonstration Project is serving postpartum families delivering at Lutheran and Denver Health.
    • OPPORTUNITY: Collaborate with Cornerstone 4 Screening/Assess/Referral.

Strategy 2

Create coordinated intake and referral system to home visitation in Jeffco, including navigators, accessible via technology and in-person, supported by a referral database, and beginning with a pilot project.

GOAL 1: Relationships and Partnerships- Build trust across programs through a home visitor connection event to improve referrals and cross-program collaboration.

2023-2024 Partner Activities

  • The HVC has collected family voice data to understand what matters most to families.
    • OPPORTUNITY: Collaborate with Cornerstone 3 on centering family voice/needs.
  • The Bilingual Family Navigator/Central Home Visitation Navigation and Referral is ongoing.
    • OPPORTUNITY: Collaborate with UPK Navigation support, Cornerstone 1.

Strategy 3

Recruit, retain, and support a qualified home visitation workforce.

GOAL 2: Shared Training- Create efficiencies around shared trainings (for both families and home visitors) and explore a DEI community of practice.

2023-2024 Partner Activities

  • Red Rocks Community College Home Visitor Apprenticeship Program is in the planning and development phase.
  • HVC has representation at the Colorado Home Visiting Coalition.
  • Circles of Security Groups and Reflective Supervision are available through Jefferson Center. The HVC will explore cross-program mental health consultation cohorts.
