Families are supported in their role as a child’s first and most important teacher with family support services that meet their diverse needs.

Develop an implementation plan and explore funding for a universally accessible home visitation system delivered through a cohesive mixed-delivery model.

2. Intake & Referral System

Create a coordinated intake and referral system for home visitation in Jeffco, including navigators, accessible through technology and in-person.

3. Workforce Support

Recruit, retain, and support a qualified home visitation workforce to meet the diverse needs of children and their families.

Advancing Strategies 2 and 3 - Creating a Coordinated Intake and Referral System; Building a Qualified Home Visitation Workforce

Goal 1: To improve referrals and cross program collaboration, the Jeffco Home Visitation Collaborative is focusing on relationships & partnerships by building trust across programs through home visitor connection events and meetings.

  • In April, the group hosted an event for 70 home visitors to get to know each other’s programs and strengthen referrals. Roundtable discussions resulted in a list of actionable items to: 1) better collaborate across programs, 2) better support families new to our community and 3) increase workforce supports.
  • Promote central home visitation navigation and the Jeffco Family Navigator to home visitors working directly with families in order to improve cross program referrals.

Goal 2:  Create efficiencies and support for the home visitor workforce through shared trainings, communities of practice and coordinated outreach efforts.
